What to expect
Patients are scheduled for 15 minute office visits during which an instant finger-stick INR is obtained as well as an assessment of therapy. Our clinical pharmacists provide the patient with dosing instructions and may renew prescriptions for warfarin and low-molecular-weight heparins on behalf of the patient's enrolling physician as part of a collaborative-practice agreement. Patients leave each visit with instant INR results, dosing regimen and next scheduled follow-up appointment. An official report with patient's INR result, assessment and dosing instructions is faxed to the patient's physician within 48 hours.
