What to expect
Patients are scheduled for a one-on-one office visit with a clinical pharmacist during which a carbon monoxide breath test, lung-age detector test and blood pressure is obtained. An individualized smoking cessation plan will be developed which incorporates patient's smoking history, past quit attempts and concomitant disease states and medications. Our clinical pharmacists may initiate prescription FDA-approved smoking cessation medications on behalf of the patient's enrolling physician as part of a collaborative-practice agreement. An official report with patient's results, progress, and initiated medication therapy is faxed to the patient's physician within 48 hours.

Patients may enroll by having their physician complete and return the enrollment form.
